You may register by clicking on "Registration Form" below and printing out the registration form. This requires the use of Adobe Reader, which can be downloaded free of charge at the Adobe Reader web site. Completed registration forms can be sent by fax to 919-962-1664 for those paying by credit card, or mailed to the address provided on the form. Phone registration is also available by calling 919-966-5136 and asking for Pain, Addiction, and the Law.
