*Must be currently enrolled in good standing in an accredited program, and a letter from the residency program director must accompany registration.

The registration fee includes course materials, breakfast, breaks, and lunch. All registrants must complete the Registration Form. Registrations will not be processed unless accompanied by full payment (check, Visa, or Mastercard). You may register by faxing your registration form with credit card information to
919-962-1664, or by mailng your form with payment to the address on the registration form.

Registration is often limited. The deadline for registration is September 1, 2008.  Accepted registrations are confirmed in writing within 10 business days of receipt. 

UNC CME recommends that you do not make air  travel arrangements prior to receiving a receipt of payment or confirmation. UNC CME is not responsible for any travel-related costs.

Cancellation Policy

Cancellations must be received in writing either by fax or mail by September 1, 2008. Cancellations received after this date will not be refunded. In order to process refunds, you must provide us with your social security number (Taxpayer Relief Act of 1997). A cancellation fee of $75 will be charged. Transfer of registration to another UNC School of Medicine conference in lieu of cancellation is not possible.

The University reserves the right to cancel or postpone any course due to unforeseen circumstances. In the event of cancellation or postponement, the University will fully refund registration fees but is not responsible for any related costs or expenses to participants, including cancellation fees assessed by hotels, airlines, rental car or travel agencies.

Although every effort is made to provide a comfortable temperature in the meeting room, it is difficult to find a temperature that is comfortable for everyone. Please bring a jacket, sweater or light wrap in case the room is too cool for your comfort. If you have special dietary needs, please contact the office of CME at 919-962-2118, no later than September 1, 2008 so that we may make arrangements to accomodate your special need.

Location and Parking

The conference will be held at the William and Ida Friday Center for Continuing Education, 100 Friday Center Drive, Chapel Hill, North Carolina. Directions will be mailed to you along with your confirmation of registration. Plenty of complimentary parking is available but be sure to park only in the designated Friday Center Lot.

Individuals are responsible for making their own hotel reservations. Please contact the hotel directly to make reservations.
